INSTRUCTIONS
1) Applicant must write their Name, Relation name, Address etc. in MALAYALAM on the print out in the space provided and hand over the same duly signed to the respective Taluk Office on the date chosen by him/her. Those who are unable to take a printout of the application should note down the application number and submit filled up application in form number 6 at the Taluk Office quoting that number.
2) The applicant can select a hearing date for appearing at the Taluk Office for producing signed application along with original/attested true copy of the document to prove his/her age,address and for taking his/her photograph.
INSTRUCTIONS FOR FILLING UP APPLICATION FORM 6
(For new enrolment in the Electoral Rolls and for shifting of name from E Roll of one Assembly Constituency to another)

1. The applicant should fill up all fields. The fields with 'star mark' are mandatory fields which are compulsory for registration.
2. For new enrolment in the Electoral Roll, the Election Identity Card number (in full) of the applicant's father/mother or relative(s) should be filled up in the respective column. Otherwise the application will not be considered.
3. For those who apply for shifting of name from one Assembly Constituency to another, the Election Identity Card number (in full) should be filled up in the respective field. The details of the Assembly Constituency in which his/her name was included earlier, the previous residential address etc should also be filled up in the respective fields.
4. The applicant can choose a date convenient to him/her for appearing for the personal hearing from the dates given below.
5. After filling up the form, the applicant should take a print out of the form and write his/her name, Relation Name, Address etc in Malayalam also in the space provided for the purpose and hand over the form to the respective Taluk Office along with copies of documents to prove Date of Birth, Residence etc on the date chosen by him for appearing for personal hearing( For applicants who apply for enrolment for the first time on attaining the age of 18 need not produce the proof of residence if he/she is applying for enrolment in the place where his/her parents/husband reside). Original documents to prove Date of Birth and Residence etc should be produced for verification of the hearing officers. ( Original certificates should not be enclosed with the application form)
6. For those who are unable to take printout should note down the Application Number generated by the Computer and submit filled up application Form 6 available at the Taluk office, quoting the application number.
7. The photograph of new applicants will be taken on the date of hearing.

For those who apply for shifting of name from one polling station to another within the same Assembly Constituency, the Election Identity Card number (in full) should be filled up in the respective field. The details of the polling station in which his/her name was included earlier, the previous residential address etc should also be filled up in the respective fields.
3. The applicant can choose a date convenient to him/her for appearing for the personal hearing from the dates given in the below.

2. The name of the person against whom the complaint is filed, his/her Part Number (Booth number), serial number in the Electoral Roll and EPIC number (if known) should be filled up.
3. The name, Part number (Booth number), Serial number and residential address of the complainant should be filled up in Part II and III of the application. The reason for filing objection should also be entered in Part IV.
4. After filling up the form, the complainant should take a print out of the form and write his/her name, Address etc in Malayalam also in the space provided for the purpose and hand over the form to the respective Taluk Office along with copies of documents if any, to prove the complaint.

After filling up the form, the applicant should take a print out of the form and write his/her name, Relation Name, Address etc in Malayalam also in the space provided for the purpose and hand over the form to the respective Taluk Office along with copies of documents to prove the necessity of corrections on the date chosen by him for appearing for personal hearing.
